Initial building design flaws can come in all forms, whether it be a leaky pipe or an imperfectly-designed structure. Over time, the issues resulting from this design flaw can lead to compounding issues that can result in mass destruction. An example of this was seen in the recent (as of writing) building collapse, where the flat pool deck caused water to stay in until it could evaporate, leading to the concrete slope becoming weak and collapsing.
